Abstract - This study focused to identify the challenge of the role that is faced by Indonesian nurse after they back from Japan and start to work as a nurse in hospital in Indonesia. The main data of this research is from questionnaire and interview, the author sent questionnaire and interviewed 10 respondents. From 10 respondents, 6 respondents have a work experience as nurse assistant and 4 respondents have a work experience as care worker assistant in Japan through IJ-EPA program. The author use role theory to analyze the challenge that is faced by Indonesian nurse after they back from Japan and start working as a nurse in hospital in Indonesia. The conclusion is all of the respondents faced difficulties and need adaptation when they start working in hospital, especially in medical action such us doing injection, infusing, etc. This is because their skill in medical action decrease after they back from working in Japan. The reason why their medical action skill decrease is because they can not do medical action when they work as nurse assistant or care worker assistant in Japan, this is related to the regulation in Japan. Because of this, when they start to work in hospital in Indonesia they can not fullfil their role as a nurse maximally.
